Requirement for Attestation by All Persons Traveling by Air to the United States
To reduce the risk of Ebola importation into the United States, every air passenger boarding an international flight into or connecting through the United States must attest (sign a statement) that they have not been in the Democratic Republic of the Congo (DRC) during the 21 days before their flight leaves for the U.S.
Travelers, including U.S. citizens and U.S. nationals, who have been in DRC within 21 days before their flight will not be allowed to board flights with U.S. destinations and should plan to remain outside the United States for at least 21 days after they leave DRC. This includes all passengers transiting through DRC on a connecting flight regardless of whether they disembark or stay on the airplane.
The Centers for Disease Control and Prevention (CDC) within the Department of Health and Human Services (HHS) issued an Order on October 22, 2026, requiring that all air travelers boarding an international flight into or connecting through the United States sign a statement (attestation) that they have not been present in DRC in the 21 days prior to boarding the flight to the United States. On October 28, 2026, CDC issued an Amended Order to clarify that the requirement for the attestation applies to all people traveling on any aircraft, including crew on passenger- and cargo-carrying aircraft involved in operation of aircraft or the positioning of crew not operating the aircraft (e.g., on "deadhead" status).
Please download, print, and sign this attestation form and present it upon boarding. Failure to complete and produce the completed attestation form will result in denial of boarding in accordance with U.S. government mandates.
